About the Role

As a Care Coordinator (Hourly Care), you’ll be the vital link between our clients and our incredible team of Healthcare Assistants. This is a dynamic, fast-paced role ideal for someone who thrives on relationship building, solving problems, and delivering a consistently outstanding service. You’ll manage daily rotas, respond to last-minute changes, and ensure our clients receive the best care possible — with warmth, professionalism, and precision.

What You’ll Be Doing

  • Build strong relationships with clients and Healthcare Assistants
  • Coordinate care rotas and manage short-notice changes
  • Respond to client enquiries and set up tailored care packages
  • Communicate with family members, local authorities, and healthcare professionals
  • Keep all care plans and client records up to date
  • Identify and respond to any safeguarding concerns
  • Maintain compliance with care legislation and internal standards
  • Support and motivate the hourly care team to deliver exceptional service

About You

You’re a natural organiser and communicator who brings energy, compassion, and professionalism to every interaction. You may already have experience in care coordination, admin, or scheduling — but above all, you’re passionate about helping others and making a meaningful difference.

You will have:

  • Excellent communication and time management skills
  • A supportive, proactive and flexible attitude
  • The ability to prioritise under pressure and stay calm in a busy environment
  • Strong working knowledge of Microsoft Office
  • A caring nature and commitment to high standards
  • Experience working in a care or healthcare setting (preferred but not essential)
